Overall Pet/Handler Team Evaluation: 

This form is to be used by team members at evaluation and
by buddies following a probationary visit.
Submit the form immediately after your visit
Your delay in returning this form will delay
your buddy’s progress in trying to complete all of the required visits.

Remember to interact with your buddy.
You are there to guide and train.
Do not simply stand on the sidelines and observe.
Offer suggestions and comments both positive and negative as needed.
At the end of the visit, be sure to offer an “exit interview”
and discuss the visit in detail (both good and bad) with your buddy.

Scoring:
Use only the scores:
NS for not suitable,
NR for not ready,
Or
A for appropriate.

Refer to the Not Suitable/Appropriate guidelines given to you.
If your buddy falls under one of those categories, score him as NS or A
If your buddy falls somewhere in between, the score should be Not Ready.
You may make brief notes in the Not Ready box if you like.
Please use the comments area to justify and explain your scoring
A score of NS in any area for pet or handler makes the team not suitable.
A score of NR tells us where the problems are,
The Board, with the input of our obedience trainers,
will decide how to handle the situation.
Always remember you are scoring the TEAM, not just the pet.
Also remember that the purpose of the probationary/training period
is to teach visiting skills and to observe the pet’s responses in a real situation.
The probationary period is not the time to teach basic obedience.
The handler should already have well developed skills;
now is the time to demonstrate how to apply those skills to a visiting situation.
Handlers without appropriate skills would be scored as Not Ready.
